Replacement SNAP Benefit Request (SNAP 38 Form)

Bossier, Caddo, and DeSoto

Bossier, Caddo, and DeSoto parish SNAP recipients who lost power for 24 hours or more following the severe storms in North Louisiana on June 16 will automatically have 60% of their monthly benefit re-loaded on their EBT cards by Friday, June 30, 2023.

To claim a food loss of more than 60%, please complete a SNAP 38 form by the July 17, 2023 deadline. Additional information supporting your claim may be required.

Other Affected Parishes

SNAP recipients in 18 other affected parishes who lost food due to power outages of 24+ hours may also apply for replacement benefits by completing a SNAP 38 form. Residents in these parishes must also submit proof of their power outage from their utility company, unless instructed otherwise HERE. The deadline to submit the SNAP 38 and proof of power outage is July 17, 2023.
